Are you having lower back pain. Maybe that pain travels into your leg. And you think you might have a lumbar disc herniation. Maybe you have seen your doctor and they say you have a slipped disc or degenerative disc disease. But do you really understand what is going on and how you got here? I am regularly surprised at the number of patients who I see who have seen other doctors for their lower back pain and still have no clue why their lumbar disc is wearing out, why they have a lumbar disc herniation, or a disc bulge/rupture. I think it is fundamental to healing (and staying healed) to first understand why a part of our body is getting injured or wearing out prematurely. Through this knowledge we can heal and stay healthy, not feeling like our health is just a roll of the dice. In this video I cover the most common reason that lumbar discs fail. Why they herniate based on movement and biomechanics. And how you need to start thinking about bending and moving your spine in a healthy way to start to heal your lower back and discs, but also to reduce your chances of having lower back issues in the future. A disc bulge is NOT the end of the world. You can often return and get back to lifting heavy without any pain. However, you MUST understand how to stabilize your spine. When a disc bulge occurs, load is no longer evenly distrusted but is instead shifted to the posterior aspects of the spine (the facet joints). While the pain from a disc bulge may eventually “burn out” in a few weeks or months, the mechanics of how the spine is instantly changed. The way in which the spine carries load from there on is affected at that particular segment and forces are now shifted to the facet joints. Overtime this overload of the facet joint could lead to arthritis and the development of pain. So if you want to continue to lift with a history of a bulging disc, you must learn to stabilize your spine. This starts with the McGill Big 3 core exercise program. Today’s video covers rehab exercises that can be done at home to help heal a disc bulge in the neck or cervical region. The neck or cervical spine includes seven vertebrae that are separated by six fibrocartilaginous intervertebral discs. The discs act as shock absorbers and also function as passive stabilizers of the spinal motion segment (2 vertebrae and the disc between them). A cervical disc herniation describes a situation where the inside of the disc (nucleus pulposus) ruptures through the outer wall of the disc. In some of these situations, neck and radiating arm pain, numbness, tingling and weakness can result. It’s also extremely important to know that disc ‘injuries’ are often observed in asymptomatic (no pain) individuals and do not necessarily say much about whether someone will experience pain in the future. Also, disc herniations can get better with conservative care, so make sure to try rehab before jumping into surgery. With all of that being said, I have several exercises demonstrated here that often help reduce pain and improve function in people with neck pain that is thought to be associated with a disc herniation. There is a tough, cartilage outer layer and a fluid, liquid “center”. Think of a jelly donut or a water balloon. Over time the outer “shell” can develop areas of weakness that the inner fluid layer can “bulge” or “escape” into. When this happens we call it a “bulged” or “herniated” disc (depending on the extent of the damage). HOW TO TREAT A BULGED DISC The goal with treatment is to get the fluid disc material out of the periphery and back into the middle where it belongs. This is best accomplished with traction, side glides, and lumbar extension activities. If a course of nonsurgical treatments (generally four to six weeks) is not effective for relieving pain from a herniated disc, lumbar decompression surgery may be considered as an option. Often a microdiscectomy (or microdecompression) - a type of lumbar decompression surgery - is used to treat nerve compression from a herniated disc The microdiscectomy procedure is usually highly successful for relieving the leg pain (sciatica) caused by a herniated disc. Although the nerve root takes several weeks or months to fully heal, patients often feel immediate relief of their leg pain and usually have a minimal amount of discomfort following the surgery. In general, if about six weeks of conservative treatment fails to relieve the arm pain or if the patient and the spine specialist determine that surgical removal of the disc is the best course of treatment, patients may consider anterior cervical decompression (discectomy). During this surgical procedure, the disc material is removed through the front of the neck and then the disc space is usually fused to keep the disc space open. Another surgical option to treat a herniated disc is a posterior cervical laminectomy, where the disc material is removed through the back of the neck. During a minimally-invasive microdiscectomy procedure, the herniated portion of the disc under the nerve root is removed. By giving the nerve root more space, pressure is relieved and the nerve root can begin to heal. The primary goal of treating a lumbar herniated disc is to reduce pain and other symptoms, not necessarily to correct the spine’s anatomy. Most of the time, symptoms alleviate quickly on their own or using nonsurgical treatments. If such treatments are ineffective after 6 weeks, more robust, long-term methods may be considered. In some cases, surgery is necessary. The first approach to treating lumbar herniated disc pain is the use of nonsurgical treatments, many of which can be administered at home and as appropriate. These treatments may include ice application, pain medications, muscle relaxants, heat therapy. If these methods are unsuccessful after a few weeks, longer-term treatment options are typically recommended. These non-surgical treatments also tend to focus on pain relief, and can include epidural steroid injections, which administer a pain relief steroid directly to the outer portion of the spine; physical therapy and exercise, which work to strengthen the spine and its supporting muscles; spinal manipulation, as administered by a chiropractor, physiatrist, or specialist to adjust spinal structures to more comfortable positions; cognitive behavioral therapy, which helps many patients cope with the emotional effects of chronic pain; massage therapy, to ease muscle tension and spasms in the lumbar spine; and acupuncture, a practice based in ancient Chinese medicine that is said to restore balance to the body’s natural flow of energy. Surgery for a lumbar herniated disc is only recommended if pain and other symptoms do not respond to nonsurgical treatments, or if low back or leg pain is so severe that it interferes with basic functions like sleeping, standing, or walking. Spinal surgery is not recommended for patients who are able to manage pain using other treatments. The most common surgery used to treat a lumbar herniated disc is a microdiscectomy, in which a small incision of between one and one-and-a-half inches is made along the spine; the muscles surrounding the spine are pushed to the side and protective membranes are removed; the herniated disc portion of the disc is removed, reducing inflammation and irritation in the area and from the nearby nerve roots. A microdiscectomy is a relatively minimally invasive procedure, and patients usually recover within 1 to 3 weeks. Residual leg pain or weakness may take a few weeks or months to heal, but most patients report immediate relief after the surgery is performed.
